Prof. M.W. Büchler takes over from Prof. C Herfarth as Head of the Department of General, Visceral and Trauma Surgery.

The appointment makes M. W. Büchler the successor of C. Herfarth. Büchler has been Director of the Department of General, Visceral and Trauma Surgery at Heidelberg University since October 2001 and Executive Medical Director of Heidelberg University Surgical Hospital since March 2003.

Büchler's surgical focus is on operations of the pancreas, especially for pancreatic carcinoma and chronic pancreatitis.

Intelligent surgery" is the keyword for physician Markus Büchler: this is the name of the research training group in which the director of the Surgical University Hospital Heidelberg and expert in general, visceral and trauma surgery is working with a particularly qualified team of young scientists to develop new methods for the operating theatre of the future.

In 2012, he was President of the German Society for Surgery.

Durch die Berufung wird M. W. Büchler zum Nachfolger von C. Herfarth. Seit Oktober 2001 ist Büchler Direktor der Klinik für Allgemein-, Viszeral- und Unfallchirurgie der Universität Heidelberg, seit März 2003 Geschäftsführender Ärztlicher Direktor der Chirurgischen Universitätsklinik Heidelberg.

Büchlers chirurgischer Schwerpunkt sind Operationen der Bauchspeicheldrüse, insbesondere bei Pankreaskarzinom und chronischer Pankreatitis.

Intelligente Chirurgie" ist das Stichwort für den Mediziner Markus Büchler: So heißt das Graduiertenkolleg, in dem der Direktor der chirurgischen Universitätsklinik Heidelberg und Experte für Allgemein-, Viszeral- und Unfallchirurgie mit einem besonders qualifizierten Team von Nachwuchswissenschaftlern daran arbeitet, neue Methoden für den Operationssaal der Zukunft zu entwickeln.

2012 war er Präsident der Deutschen Gesellschaft für Chirurgie.

Focus on kidney and pancreas transplantation

Integration of kidney transplantation into the visceral-surgical transplantation activity (liver and pancreas)

Organ transplantation is an important interdisciplinary focus of Heidelberg University Hospital. In order to provide patients and relatives with a modern competence centre for the transfer of organs, these activities have been combined under the name Transplantation Centre Heidelberg .

As early as 1967, the first kidney transplant in Germany was performed in Heidelberg. Since then, more than 3100 kidneys have been transplanted. With the transition of kidney transplantation under the umbrella of visceral organ transplantation, all internal organs of the abdominal cavity are now under common management. Approximately 140-160 kidney transplants, many of them also by living donation, are successfully performed in Heidelberg every year.

In the meantime, about 30-40% of these are living kidney donations. To date, 664 living kidney transplants have been realised. A special focus is also transplantation against barriers (blood group and tissue antibodies), which is carried out in Heidelberg with great expertise and success.

Since the introduction of pancreas transplantation in 1992, approx. 100 pancreas transplants have been realised in Heidelberg.

Integration der Nierentransplantation in die viszeral-chirurgische Transplantationsaktivität (Leber und Pankreas) 

Die Transplantation von Organen ist ein wichtiger interdisziplinärer Schwerpunkt des Universitätsklinikums Heidelberg. Um Patienten und Angehörigen ein modernes Kompetenzzentrum für die Übertragung von Organen zur Verfügung zu stellen, wurden diese Tätigkeiten unter dem Namen Transplantations-Zentrum Heidelberg  zusammengefasst. 

Bereits 1967 wurde die erste Nierentransplantation in Deutschland in Heidelberg durchgeführt. Seither wurden mehr als 3100 Nieren verpflanzt. Mit dem Übergang der Nierentransplantation unter das Dach der viszeralen Organtransplantation sind jetzt alle inneren Organe des Bauchraumes unter gemeinsamer Leitung. Ca. 140-160 Nierentransplantationen, viele davon auch durch Lebendspende, werden jährlich in Heidelberg mit Erfolg durchgeführt.

Inzwischen entfallen hiervon ca. 30-40% auf eine Nierenlebendspende. Bis heute konnten so 664 Lebendnierentransplantationen realisiert werden. Ein besonderer Schwerpunkt bildet hier auch die Transplantation gegen Barrieren (Blutgruppe und Gewebeantikörper), die in Heidelberg mit großer Expertise und Erfolg durchgeführt wird.

Seit Einführung der Pankreastransplantation im Jahr 1992 wurden ca. 100 Pankreastransplantationen in Heidelberg realisiert.

Behandlung und Erforschung von Erkrankungen der Bauchspeicheldrüse auf konstant hohem Niveau: Chirurgische Universitätsklinik Heidelberg von Fachgesellschaft wiederholt zertifiziert

Bei Erkrankungen der Bauchspeicheldrüse ist die Chirurgische Universitätsklinik Heidelberg und das dort angesiedelte Europäische Pankreaszentrum eine hervorragende Adresse. Das hat die Deutsche Gesellschaft für Allgemein- und Viszeralchirurgie (DGAV) nach einer umfassenden Prüfung erneut bestätigt. Die Klinik behält damit den Titel „Exzellenzzentrum für Chirurgische Erkrankungen des Pankreas“, in dem Patienten nach vorgeschriebenen hohen Qualitätsstandards bei gleichzeitig großer Erfahrung des Behandlungsteams versorgt werden.

Das Zertifikat als Exzellenzzentrum erhielt das Europäische Pankreaszentrum in Heidelberg 2012 von der DAGV, weitere Qualitätsprüfungen sind alle drei Jahre vorgesehen. Die erste Folgeprüfung in diesem Jahr ergab: In Patientenversorgung, operativen Eingriffen, aber auch der Erforschung von Erkrankungen der Bauchspeicheldrüse hält die Klinik ein konstant hohes Niveau. „Diese Bestätigung unserer bisherigen Arbeit motiviert, uns in Diagnostik und Therapie konsequent noch weiter zu verbessern“, sagt Professor Dr. Markus W. Büchler, Geschäftsführender Direktor der Klinik.

Treatment and research of pancreatic diseases at a consistently high level: Heidelberg University Hospital repeatedly certified by professional association

Heidelberg University Hospital and the European Pancreas Centre based there are an excellent address for diseases of the pancreas. The German Society for General and Visceral Surgery (DGAV) has confirmed this again after a comprehensive audit. The clinic thus retains the title of "Centre of Excellence for Surgical Diseases of the Pancreas", where patients are cared for according to prescribed high quality standards while the treatment team is highly experienced.

The European Pancreas Centre in Heidelberg received its certificate as a centre of excellence from the DAGV in 2012, and further quality audits are scheduled every three years. The first follow-up audit this year showed that the hospital maintains a consistently high level of patient care, surgical interventions and research into pancreatic diseases. "This confirmation of our work so far motivates us to consistently improve even further in diagnostics and therapy," says Professor Dr. Markus W. Büchler, Managing Director of the clinic.

2001 - Gründung des Klinischen Studienzentrums Chirurgie (KSC)

Ziel ist es, die beste Therapieform Patienten zu finden.

Bislang basieren nur 10 bis 15 % aller Therapien in der Chirurgie auf den Ergebnissen randomisiert kontrollierter klinischer Studien. Mit dem Ziel, diesen Anteil deutlich zu erhöhen, wurde am 1. Oktober 2001 das Klinische Studienzentrum Chirurgie (KSC) gegründet und der erste Heidelberger Studienpatient am 6. Mai 2002 in eine randomisiert kontrollierte Studie (RCT) eingeschlossen.

Eine randomisiert kontrollierte Studie (RCT) vergleicht in der Regel zwei unterschiedliche Ansätze zur Behandlung eines Krankheitsbildes, z.B. zwei  Arzneimittelregime, eine medikamentöse Therapie versus eine Operation oder auch zwei unterschiedliche Operationstechniken. 
Hierdurch werden zwei Gruppen von Patienten gebildet, bei welchen nach Durchführung der Therapie Erfolge untersucht und verglichen werden können. 

Ziel ist es, die beste Therapieform Patienten zu finden. Ohne die Durchführung solcher Studien ist eine moderne, auf wissenschaftsbasierten Grundprinzipien ruhende Krankenversorgung (evidenzbasierte Medizin = EbM) nicht möglich. 
 
Das KSC führt chirurgische Studien zum Vergleich operativer Verfahren und Auftragsstudien der pharmazeutischen Industrie zur Entwicklung neuer Medikamente in der Chirurgie (z.B. Thromboseprophylaxe, Antibiotika-Therapie, Schmerzbehandlung etc.) durch. 
 
Im Studienzentrum werden Patienten von speziell ausgebildetem Studienpersonal betreut. Dieses setzt sich zusammen aus besonders qualifizierten Ärzten, Krankenschwestern und Krankenpflegern sowie Arzthelferinnen. Durch einen Studienrufdienst steht an 365 Tagen im Jahr ein Ansprechpartner zur Verfügung, sodass vielen Patienten die Teilnahme an einer Studie ermöglicht werden kann.

The aim is to find the best form of therapy for patients.

So far, only 10 to 15 % of all therapies in surgery are based on the results of randomised controlled clinical trials. With the aim of significantly increasing this proportion, the Clinical Study Centre Surgery (KSC) was founded on 1 October 2001 and the first Heidelberg study patient was included in a randomised controlled trial (RCT) on 6 May 2002.

A randomised controlled trial (RCT) usually compares two different approaches to the treatment of a clinical picture, e.g. two drug regimes, one drug regimen, one drug regimen, one drug regimen and one drug regimen.For example, two drug regimes, a drug therapy versus an operation or two different surgical techniques. 
This creates two groups of patients in whom success can be examined and compared after the therapy has been carried out. 

The aim is to find the best form of therapy for patients. Without conducting such studies, modern health care based on scientific principles (evidence-based medicine = EbM) is not possible. 

The KSC conducts surgical studies to compare surgical procedures and commissioned studies by the pharmaceutical industry to develop new drugs in surgery (e.g. thrombosis prophylaxis, antibiotic therapy, pain treatment, etc.). 

At the study centre, patients are cared for by specially trained study staff. This consists of specially qualified doctors, nurses and medical assistants. A study call service provides a contact person 365 days a year, so that many patients can participate in a study.

The ZPM ensures the smooth running of patient admission and surgical coordination.

With the founding of the ZPM, the preoperative length of stay was shortened in such a way that the optimisation of length of stay required by the introduction of DRGs could be implemented. The ZPM has proved its worth in its bridging function between medical referring physicians and the surgical polyclinic on the one hand and management in the inpatient area on the other. It has established itself as an effective control instrument in the department, which ensures a smooth process of patient admission and OR coordination.

Das ZPM gewährleistet den reibungslosen Ablauf der Patientenaufnahme und OP-Koordination

Mit der Gründung des ZPM wurden die präoperativen Liegezeiten so verkürzt, dass die mit der Einführung der DRG´s erforderliche Optimierung der Liegedauer umgesetzt werden konnte. Das ZPM bewährt sich in seiner Brückenfunktion zwischen ärztlichen Zuweisern und Chirurgischer Poliklinik einerseits und Management im stationären Bereich andererseits. Es hat sich als wirkungsvolles Steuerungsinstrument in der Abteilung etabliert, das einen reibungslosen Ablauf der Patientenaufnahme und OP-Koordination gewährleistet.

Keine andere deutsche Klinik hat mehr Erfahrung bei der Behandlung von Bauchspeicheldrüsenentzündungen und -krebs

Im vergangenen Jahr wurden an der Abteilung für Allgemein-, Viszeral- und Transplantationschirurgie insgesamt 626 Eingriffe vorgenommen, im Jahr davor waren es 532. "Neben einer bestimmten Mindestanzahl an Operationen und der notwendigen Infrastruktur zur Diagnostik und Therapie von Erkrankungen des Pankreas setzt eine Zertifizierung als Exzellenzzentrum vor allem eine interdisziplinäre Behandlung der Patienten voraus.

No other German hospital has more experience in the treatment of pancreatitis and pancreatic cancer

Last year, a total of 626 operations were performed at the Department of General, Visceral and Transplant Surgery, compared to 532 the year before. "In addition to a certain minimum number of operations and the necessary infrastructure for the diagnosis and treatment of pancreatic diseases, certification as a centre of excellence requires, above all, the interdisciplinary treatment of patients.
